Our goal is for partners and the community to come together to think, plan and act in innovative, collaborative and coordinated ways which will ensure that individuals and communities are empowered to maintain and improve their health and wellbeing.

Our partners provide health and community services within the Outer East Metropolitan area of Melbourne including the municipalities of Knox, Maroondah and Yarra Ranges.
