Service Integration

Care coordination aims to place consumers at the centre of health service delivery. Service providers work together to  ensure that they have access to the health services they need, when they need them, opportunities for early intervention, health promotion and improved health outcomes. The practice of service coordination particularly supports more effective ways of working with people with complex and multiple needs.

The OEHCSA is working with member agencies to strengthen collaboration and integration, improve service delivery, reduce duplication and improve the client journey through the healthcare system.
