Electronic Shared Care Planning
In 2010 nine PCPs from across Victoria received funds from the Victorian government to complete an e-Care Planning Project. The project focussed on progressing the most complex layer of care planning: shared care planning, that is, coordination of inter-service/inter-agency care planning.
In this phase an electronic care planning tool was developed which enabled different agencies to create a shared e-care plan. The e-care planning tool allowed multiple care providers to communicate in real time with each other, ensuring they were all up to date regarding the care of their shared client.
Phase 2 was supported by funding from the Commonwealth and Victorian Governments under the Home and Community Care (HACC) Program. Building on Phase 1, this phase focused primarily on supporting the change management process required to embed the use of the tool into practice. Organisations involved were providing shared care for client age group (over 65).
